Avoidance=
Do you try to ignore thoughts and feelings from your past?
When you get upset, anxious, or stressed do you shut down your feelings so that you don’t have to cope with them?
Do you sometimes feel like a pressure cooker inside from holding in your feelings?
Do you have unexplained outbursts, meltdowns, or emotional distress and not able to explain why?
Dissociation=
Have you ever experienced feelings of being outside your body, like the world is not real, extreme memory loss, or emotional numbness?
Have these episodes ever happened during childhood, or during stressful moments?
Have these episodes ever happened shortly after a traumatic experience or trigger from the past?
Self-harm=
Have you ever self-harmed?
What were you feeling when you self-harmed?
What release do you feel after harming yourself?
What has happened that made you turn to self-harming?
Addictions=
Are there substances/behaviors you use that give you pleasure?
Do you crave, long for, or hurry to get to this substance/behavior?
Do you crave the substance/behavior when feeling stressed, sad, or anxious?
Does the money you spend on this substance/behavior hurt your financial safety?
Has anyone close to you expressed a problem or concern with you using this substance or performing this behavior?
Are you able to refrain from using the substance/behavior?
What feelings are you using the substance/behavior to calm or numb?
What caused you to have those feelings?
Suicidal Thoughts/Attempts=
Have you ever wished you were dead or had no desire to stay alive?
Did you ever attempt to commit suicide?
Do you still have these thoughts in adulthood?
What happened to make you feel like you want to die or not have the desire to live?
Are there things, people, or experiences that bring you joy and give you are reason to want to live?
